Activity Aide/Support Staff

Department: Adult Day Health

General Description: Per diem staff needed to provide the necessary supports for program participants to fully engage in activities of the Adult Day Health program. Support staff report directly to and receive weekly and/or as needed support and supervision from their supervisor within the Adult Day Health program.

Minimum Qualifications: High School Diploma or equivalency. Minimum of at least one year of work experience in the field of providing supports to the geriatric population in a health-care or social service setting. Certification in First Aid/CPR. Ability to successfully pass extensive background checks including BCI, CORI, OIG.

Job Responsibilities: Assist Activity Director and/or Coordinator in providing a comprehensive program of activities tailored to meet the needs of participants. Independently carry out therapeutic activities with participants under the guidance of the Activity Director or Coordinator. Ensures the safety and well-being of participants and promotes optimum health, independence and self-care. Assist people to participate in meaningful recreational and social activities they prefer. Set up and operate equipment, lead games, singing, discussions and craft opportunities. Teach individuals to perform tasks necessary to function as independently as possible. Assist in the implementation and evaluation of the participant’s written plan of care. Assist with ambulation as needed. Note changes in participant’s physical or emotional health and communicate changes to nursing or social service staff. Assist in serving participant’s prescribed meals i.e. type of diet, consistency, special dishes and/or utensils as ordered. Receives participants upon arrival, completes attendance sheets and ensures participants utilize appropriate mode of transportation when leaving program. Assists with activities of daily living i.e. toileting, feeding, grooming, walking, transfers and dressing as needed. Practices universal precautions as needed. Support individuals to perform tasks necessary to participate and contribute in activities of their choice. Contribute to the development of support programs and implement objectives. Complete documentation required for individual programs. Act as a role model, meeting expectations of the community for social norm and dress. Maintains confidentiality of participant. Complete required trainings and maintain certifications.
