General Description: Part-time, 32.5 hours per week position, Mon.-Fri. needed to provide supports to individauls served in our Day Habilitation program in Middleboro, MA. 8:30am-3:00pm.
Minimum Qualifications: High School diploma or GED, one year of experience in the field of developmental disabilities and/or mental health setting. Safe driving record, current drivers' license, ability to pass criminal background checks.

General Description: RN needed for 10 hours per week/flexible schedule to provide medical oversight to adults with developmental disabilities who participate in our residential supports program and Shared Living program.
Minimum Qualifications: RN licensure in the state of RI. Experience in the field of developmental disabilities/community based health care. Ability to teach/train and monitor direct support staff and Shared Living providers in terms of medical oversight. Ability to pass criminal background checks, possess a valid drivers' license. Ability to work independently and as a member of the interdisciplinary team.
Job Responsabilities: Assess the physical, emotional and behavior needs of inds. Organize and maintain the medical records of inds. Complete required documentation. Create plans of care. Monitor medications and any medical tx. Teach medication class and signs/symptoms of illness classes on a quarterly basis.

General Description: Part-time position needed to train and supervise HBTS treatment staff in implementation of HBTS Treatment Plan, develop individual treatment plan and relevant measurement systems; design and assist in the management of research projects; participate in program development activities as designated by the director; provide parent training, consult to public schools. Minimum Qualifications: The qualifications listed below are representative of the knowledge, skill and/or ability required. -Must be a Rhode Island licensed health care professional eligible to provide clinical supervision as outline in State of Rhode Island Department of Human Services Certification Standards for Providers of Home Based Therapeutic Services. The LCSW will be supervised by a LICSW. A minimum of 3 years working with children and families in a home based setting preferred. Minimum of three years working with children or adults with special health care needs.

Job Responsabilities: As specified in the Adult Foster Care Provider Agreement and subcontractor agreement including but not limited to: providing nutritionally balanced meals, assistance with personal hygiene as needed, assistance with selection and purchases of personal clothing and accessories, money management, encouragement in maintaining community relationships, assist with school, day services, or employment of individual, ensure the provision of 24 hour support/unless otherwise specified in support plan, and use own vehicle to transport individual to medical, recreational, and other necessary trips.

Department: Adult Foster Care
General Description: Full-time, 35 hour position available within the Attleboro, Taunton, Rehoboth areas. Provide medical assessment and oversight to the participants of the Adult Foster Care program. Coordinate medical services for participants of the program and work closely with the care givers to ensure the medical needs of participants are being met. Complete intake assessments to determine medical needs.
Minimum Qualifications: General Qualifications
• Bachelor’s Degree in Nursing and RN Licensure
• Minimum of two (2) years equivalent work experience directly related to aforementioned duties and responsibilities strongly preferred
• Knowledge of community system serving adults with disabilities
Job Responsabilities: The nurse reports directly to and receives weekly and/or as needed support and supervision from The Director of Adult Foster Care Services.
The responsibilities of the registered nurse will include and not be limited to:
1. Evaluation of potential participants and foster families
2. Liaison to doctors, dentists, hospitals and other support services as necessary. Participates in the orientation of participant’s to the program and family homes and trains caregiver on any medical health related care issues.
3. Completion of the medical components of the participant’s assessment form
4. Monitoring of the participant’s health status and completion of semi-annual participant health status report and patient care plan
5. Planning, implementation, and evaluation of foster family unit
6. Provision of case management to the new family unit
7. Review and supervision of personal care services at least every 60 days.
8. Teaches participants/foster caregivers about the health consequences of various risky behaviors.
9. Oversees the medication administration certification of the employees
10. Reviews significant event reports and medication error forms with follow up action as necessary.
11. Receive and transcribe medical orders from physicians and ensures implementation
12. Communicates effectively and professionally.
13. Responsible for participation in Quality Management as relates to health and nutrition in family homes.
14. Performs related duties as assigned.

Minimum Qualifications: •Be at least 19 years of age; have a high-school degree or equivalent, and one year of supervised experience working with children, or
•Have at least 3 years of experience with adults with developmental disabilities to work as home-based workers, or
