Remote Supports & Housing Coordinator Job #1242

General Description

 

The Remote Supports and Monitoring Coordinator is responsible for the departmental operations associated with the provision of services to individuals served receiving Remote Supports and Monitoring.

 

General Qualifications

 

Bachelor degree in human services or related field.  At least two years of experience working with individuals with developmental/intellectual disabilities and their families.  Experience in advocacy and supervision preferred.  In lieu of a degree, minimum five years of experience working with individuals with disabilities.  Ability to pass extensive background checks, safe driving record, current driver’s license and motor vehicle liability insurance.

 

Job Responsibilities

 

·         Act as a liaison between the Remote Support agency (Safe-in-home) and the Arc.

·         Supervises Individual Supports staff that work with individuals receiving Remote Supports and oversees their activities.

·         Participate in an on-call role should the Remote Monitoring team need to speak with an agent of the Arc.

·         Will work with Mansfield Housing Authority to facilitate finding housing for individuals, according to approved requirements.

·         Will maintain a waiting list with Mansfield housing and maintain communication as necessary to provide up to 50 housing vouchers to start.

·         Will research suitable housing based on individual’s needs.

·         Works collaboratively with team members to facilitate individualized person centered supports.

·         Supervise assessment, development and implementation of Remote Support strategies in conjunction with other interdisciplinary team members.

·         Assess program services and modify and develop new services to meet identified needs of individuals and program efficiency.

·         Works closely with the human resources department in regards to recruiting employees, interdisciplinary action as needed and collaborates on staff training and evaluation

·         Establishes and maintains a working environment conducive to positive moral, individual style, quality, creativity and teamwork.

·         Ensures provision of services that are in compliance with state/federal regulations and timelines, including compliance with licensing and certification entities. 

·         Monitors Remote Supports accomplishments and prepares reports to evaluate contractual compliance and internal objectives, including satisfaction surveys, performance objectives and departmental reports.

·         Identifies community resources and fosters community collaboration.

 

·         Liaison to DDS regarding ongoing Remote Supports needs and ensuring continuity of services and serves as an advocate for all individuals in the program.

·         Represents the Arc of Bristol County d/b/a proAbility throughout external community groups.

·         Works with individuals, family members, The Arc team, state and other agency representatives to promote and develop natural community based supports and opportunities.

·         Maintains necessary certifications/training

·         Other duties as assigned.
