Agency With Choice Service Navigator
General Description:
The AWC Service Navigator coordinates services within the Agency with Choice Program, participates in relevant training, and reports to the Director of Family Support.
General Qualifications:
Bachelor’s Degree in Social Work or related field (preferred) and two to five years of experience in the field, working with individuals with developmental and intellectual disabilities and with their families. Other qualifications include working knowledge of resources, benefits, and services; good communication and organization skills; ability to make sound clinical decisions; and a commitment to working as a team member.

● Encourage people with disabilities to live their best lives by supporting their life vision and goals.
●Work with each person participating in AWC services to create a Support Team to help carry out chosen vision and goals.
●Foster self-growth, community participation, and personal networking opportunities using a strengths-based approach.
●Create meaningful systems for possible employment, volunteer options, and personal connections as people pursue their life aspirations.
●Develop with AWC participant/family a proposed AWC budget.
●Promote community connections by helping to identify and share contact information for local, generic community resources, and community organizations that may be helpful to the participant/family to address their support needs, including recruitment of potential Direct Service Staff, and providing opportunities for new experiences and relationships.
●Maintain regular contact (home visits, phone calls, emails) with AWC Participant and their family.
●Assist in recruiting support staff and help in all activities related to the employment process.
