Adult Family Care (AFC) Coordinator-Attleboro Area

General Description:

The Adult Family Care (AFC) Coordinator works as part of a Multidisciplinary team (Vice President, Director, AFC Program Manager, Nurses, and Coordinators) to provide AFC Services to meet the care management needs of the members.  The AFC Coordinator participates in required and other relevant training opportunities and reports directly to the Director of Adult Family Care. 

General Qualifications:

Bachelor’s degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years of recent experience working with elders or adults with disabilities; or a bachelor’s degree and two year of clinical experience in the care of elders or people with disabilities. 

Excellent interpersonal and communication (written/verbal) skills; good organizational skills; and commitment to working as a team member are needed.  Annual physical and bi-annual TB screening are required.

Job Responsibilities and Expectations:

·         In conjunction with the AFC Director, assess appropriateness of referrals for program by evaluating potential participants for the AFC program. Obtain a social history and conduct a psychosocial assessment and evaluation of potential members.

·         Provide ongoing support and assessment of members’ needs through monthly home visits.

·         Participate in the development, implementation, and ongoing review of the AFC plan of care.

·         Participate in multi-disciplinary team meetings with Director and AFC RN.

·         Collaborate with community resources and services, state departments, and disability or health related specific services to determine how best to meet the needs of members, their caregivers and families.

·         Participate in the individuals’ ISP/IEP meetings, care plan meetings, and any other relevant meetings.

·         Assist to select, train, assess, and support AFC Caregivers.  Conduct annual home safety assessments of the AFC qualified setting.  Ensure Caregivers’ compliance with fire and safety procedures.

·         Complete a progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.

·         Provide assessment of, support, and guidance for AFC Caregivers to ensure compliance with needed training/certifications, care and support of member, community involvement, and needed documentation as required by MassHealth regulations.  Ensure timeliness of caregiver log and invoice documentation as it relates to both program and billing requirements and submit absence days tracking form coinciding with invoice schedule.

·         Participate in discharge planning and implementation as needed in conjunction with AFC RN.

·         Protect the rights and dignities of individuals supported and extend those rights and dignities to family members, caregivers, or guardians.  Represent The Arc of Bristol County, Inc., while in the community in a positive and professional manner, consistently communicating the agency’s philosophy and mission.  This includes interactions with all stakeholders: individuals, family members, caregivers, guardians, community partners, etc.

·         Report changes in the member’s condition to their AFC RN.

·         Provide timely assistance and responding to urgent or emergency needs of the member.

·         Follow all agency and program specific policies and procedures.

·         Serve as staff on-call in a rotation system within the organization.  Serve as a back-up support staff in emergencies for the AFC program.

·         Maintain the accuracy and timeliness of submission of all documentation as related to members within AFC services (MassHealth required forms; status notes; emergency information sheets; releases of information; safety assessments and plans; human right training; individuals’ health, housing social security benefits; etc.)

·         Maintain accuracy and timeliness of submissions of bi-weekly timesheet and monthly mileage forms.

·         Other duties as assigned

Full Time 40 hours per week, base rate $23.00/hour
