What We Do
Individual Supports provide home and community supports for individuals with intellectual and/or developmental disabilities who reside in their own homes or apartments. The program aims to help adults thrive while maintaining their independence and autonomy by creating personalized plans, fostering skill development, and supporting individuals in learning about and engaging with their community.

Eligibility
Adults aged 22 and older with disabilities who can live independently with support tailored to their individual needs must be referred by the Massachusetts Department of Developmental Services (DDS).

Services and Supports
Individual support plans are person-centered, tailoring the services and supports to each person.
Services and supports can include:
-Daily living tasks like grocery shopping, cleaning, and running errands
-Scheduling and attending medical appointments
-Planning recreational activities and outings
