proAbility Shared Living Arrangements (SLA)

What We Do

proAbility provides individuals the chance to live with a host family. These arrangements foster a family environment, supported by the Shared Living provider, who offers housing and care. Together, they create a shared home as the individual progresses in their service plan.

  • Eligibility

    Individuals must have a disability, be 21 years old, and be referred by theDepartment of Behavioral Healthcare, Developmental Disabilities & Hospitals (BHDDH).

  • Services and Supports

    The Shared Living Provider will help the individual with:

    • daily activities like cooking, transportation, and personal care.
    • taking part in other services, like community-based supports.
    • medical appointments.
    • taking part in familial activities.

    Together, the person and provider will create a happy, supportive home.
