Shared Living
Shared Living Services are provided to individuals who need assistance or can no longer live independently. The individual lives with a provider in the provider’s home. Shared Living offers an alternative to traditional 24-hour group homes, providing a more individualized and family-like living environment.

Eligibility
Adults (22 years and older) with intellectual and developmental disabilities and referred by the Massachusetts Department of Developmental Services (DDS)

Shared Living Potential Provider Responsibilities
-Provide services in the Provider's home
-Supervise and support the physical and emotional wellness of individuals
-Assist with activities of daily living (ADLs) like cooking, meal preparation, laundry
-Provide a clean, safe, and supportive home environment
-Assist with medication administration as needed
-Schedule and accompany individuals to medical and healthcare appointments
-Support personal goals outlined in the Individual Service Plan (ISP)
-Maintain reliable transportation for appointments and activities
